This might not be bug. It is feature. Parameter -o have one feature,
page number replacement $04d, without it on filename, all pages will be
have same name and file will be written top.

What was scanadf parameters. Without, there is no real idea of possible
reason.

At least this happened for me.

from scanadf man page:

The -o or --output-file option specifies a format string used to
generate the name of file to write the image data to. You can use %d
replacement in the output file name; this will be replaced with the
current page number. The default format string is image-%04d.
