I ran into this problem after update to 25.04
I found this line in /var/log/syslog

gsd-media-keys[4863]: Could not launch 'xdg-terminal-exec %f': Program
‘xdg-terminal-exec’ not found in $PATH

After installing xdg-terminal-exec (sudo apt install xdg-terminal-exec),
the Ctrl-Alt-T shortcut is working again.
