I see the same errors as the OP, although I see somewhat different index 
numbers:

I am not currently trying to use the card, but I could hook up an antenna, if 
there's a patch to test.
This is on older hardware than the OP's, with an AMD Athlon(tm) Dual Core 
Processor 4850e. LMK if you'd like more info.
