I've seen this exact error as well.  I can say in the instance where I
was asked to assist with troubleshooting the issue it was caused by
performing the release upgrade when the root account had been enabled
and the password had been allowed to expire.  After changing the root
password so that it was no longer expired the release upgrade completed
without issue.
