Thank you for the extra information. Looking at the PNG file you've attached, I see a bunch of I/O error messages being displayed while trying to access the device. I'm not an expert here, but it looks to me like there may be a hardware problem involved? Why are you seeing so many I/O errors? Are they eventually causing the device to be removed (probably because sg3-utils cannot access it)?
