I have trouble pinpointing the actual problem. Did you want to say
"15-25 seconds" instead of "minutes"?

The only unusual behaviour I see in your log is that the very first run
of usb_modeswitch takes roughly 40 seconds. Afterwards, everything seems
to be fine, modem and storage.

I suppose the later disconnect and reconnect was caused by you
unplugging and re-plugging the modem, right? It's at 09:50:58.
