The actual issue that breaks your case is the "failed to open host usb device 2:6". I'd first of all assume it might be an apparmor deny or something similar. Could you track dmesg -w while you try to attach the device. Are there any denies reported int hat regard? Further could you please make sure that $ virsh nodedev-list lists your device (the one you try to add) and also report $ virtsh nodedev-dumpxml <id> here just for completeness.
