I just checked mousing over the min/max/close buttons. I repeatedly moved my mouse on to and off of the min button. The title bar disappears after 5-13 times I do this. When changing the text of the title bar (switching tabs in firefox) it happens exactly every 7 times. But the problem is definitely not limited to a change of title bar text, it's any redraw.
