Public bug reported:

There is a bug in GRUB where it will not write the boot loader to the
disk.  It simply says "Fatal Error" and then exits.  Further attempts
will result in the same error message.

To fix this error, I can take the drive out and put it into a windows
machine, switch it to Dynamic Disk, then format it.  When I place it
back into the original computer, GRUB will install without any problem.

I hope that will provide a clue to someone more knowledgeable as to what
the real problem is.

To reproduce this problem.  Get a computer with UEFI boot.  In my case
it was a Dell PowerEdge R410.  Install OPNsense firewall on it:

Next, install Ubuntu 18.04 and you will get the GRUB error.  It's a
fatal error and you cannot continue!
