There isn't anything out of the ordinary in the compute configuration so it's likely the instances came up as SHUTDOWN but would not start. Again, I will confirm this. Regarding the replication test in comment #8, I believe those two instances were booted from images. A critical component of the failure scenario is booting the instance from a volume.
