Public bug reported: Frequently, when I insert a USB hub with two HID devices (Logitech wireless keyboard & mouse), one of them fails to initialize, and I get a "submit error report" dialog. Removing and reinserting the hub usually fixes the issue. I found a crash report and attached it, but not sure if it is for the same issue or for something else.
