So there appear to be two moving parts here. a) If building against a version of libmysqlclient without make_scrambled_password, then https://github.com/NigelCunningham/pam- MySQL/commit/2f6837b5fb8ef4f5d8a561f3bf82b36cc79e77f4 is needed. This commit is present in 17.04 but not earlier releases.
b) libpam-mysql had a b-d on libmysqlclient15-dev. This was fixed in Ubuntu in 0.7~RC1-4ubuntu2 for 16.04 and in Debian in 0.7~RC1-4.1 (which is present in 16.10+).
