You have been subscribed to a public bug: On HP Z640 Workstations running ubuntu 16.04 with any official ubuntu kernel newer than 4.4.0-21-generic USB devices do not work.
They appear to be listed correctly via lsusb and connects and disconnects are logged but keyboards, mice, storage devices, ethernet adapters do not work at all when they are connected directly via any USB ports on the machines or through USB hubs. All ubuntu packages are up to date. The BIOS version is the latest (2.29). The ubuntu 4.4.0-21.37 kernel works correctly as do older kernels from ubuntu 16.04. Vanilla kernel.org kernels that I've tried such as 4.4.30 and 4.4.38 work correctly.
