I confirm this bug, using totem 2.20 with gstreamer 0.10.14. Refresh rate allways switches to 60hz on fullscreen toggle (moreover 60hz is broken for me due to some bug with xorg intel drivers: the display is squashed to the right 3/4 of the screen at 60hz, producing a distorted image; works fine at 75hz) - so there is no way of seeing fullscreen movies at any refresh rate other than 60Hz, apart from the obvious remark that totem shouldn't be causing this.
