Public bug reported:

I have a USB 16.04 LTS installer (supplied by you). I'm installing to a
Dell Inspiron with a completely formatted 500GB drive. I've check with
badblocks and the disk is fine. The installer fails with an IO error
saying that the DVD\CD drive may need cleaning.
