Public bug reported:

LUKS boot for encrypted root systems was broken starting with the
3.19.0-49 kernel and all subsequent 3.19 kernels due to an
incompatibility between upstream kernel patches not being fully applied
and the current vivid cryptsetup version (2:1.6.7-0vanir1~15.04).

This is a documented kernel bug, but Ubuntu has not picked up patches.

Current workaround is booting off last working kernel
(3.19.0-47-generic). I cannot upgrade to 16 until fix is confirmed as
it will force de-install of last current working kernel.

** Affects: libpam-mount (Ubuntu)
     Importance: Undecided
         Status: New
