Hey guys, 
I took a video of the output so that hopefully you can pause at points to get 
everything you need that is on screen (I don't think it would fit in one photo 
with my terrible phone camera).

I downloaded and installed kernel 4.5 and had the same issue.

I really appreciate the work you guys are putting in. I don't know if it
matters but the issue happened around the time I was having to plug in
and unplug various data drives in order to get some data off a RAID
array (on another machine) that failed recently.
