I spent some time debugging the issue, and eventually wrote a kernel patch which adds support for this card: http://git.linuxtv.org/cgit.cgi/media_tree.git/commit/?id=8c47311d34eccedb06bc60fc9435a53bd4aff392
The patch has been accepted into the linuxtv git project, and will eventually make its way into Linus's kernel.
