Fine, but we are still waiting for answers to the above questions, see comment 19. Like I said: there may in fact be a bug here, but we need to understand if the root volume was intended to be multipathed (i.e. configured in the OS as a multipath group), and if that failed to happen during install (or a subsequent reboot), and if the root volume was part of the failover tests also.
