Ah that one I was thinking of is registered with the Squid team as #1900

That would account for an extra long shutdown resulting in "failed" then being 
forcibly killed.

And then there is bug #1845 - 'the impatient user/script' bug

Where shutdown_lifetime needs to be configured properly when a quick shutdown 
is needed (high traffic servers or other low-downtime/fast-cycling systems)
