After some research (please look at http://www.spinics.net/lists/linux-bluetooth/msg57515.html) I end up with the firmware correctly loaded from a file called BCM43142A0-0489-e062.hcd. But even this way, I need to reboot Linux several times in order to get the BT adapter fully working. Sometimes I get lucky and it works at the very first try, but it does not happen very often.
