We've since stopped making alternate CDs for any of the recent releases you may need to upgrade to (last was 12.04, and one would usually upgrade *from* there to another release), so I think it's safe enough to close this bug as "Won't Fix".
If anyone runs into this kind of issue again, please file a new bug making sure to mention exactly which releases are being upgraded from and to (just like it was done above), so we can have as much data as possible to be able to look into the issue.
