Please answer the questions / provide the following info so we can continue 
debug on our site:
1. do you have a Mellanox Firmware version that works?
2. what kernel version did you use? We had a fix for interface naming issue in 
Ubuntu-3.13.0-35.61 - that may fix it.

What I think that we need in addition is, take the following info before and 
after the modules reload:
1. lsmod | grep mlx4
2. grep . /proc/net/bonding/bond*
3. grep . /sys/module/mlx4*/parameters/*
4. ifconfig -a
5. ethtool -i <mlx4_ifs>
6. grep . /sys/class/net/*/* 2> /dev/null

and these we need only once, after the driver was reloaded:
1. provide your /etc/network/interfaces and additional included files if any.
2. dmesg
3. uname -r
4. grep mlx /etc/modprobe.d/*
5. cat /etc/issue
6. ofed_info

Please note that we will need HP to provide these answers, as they have
the m400 hardware that displays the problem
