How come do we not see this when the updated QtBearer plugin isn't
installed?

What this tells me is something is asking NM to activate a connection
when it already is; so rather than looking at NetworkManager, I'd look
at whatever is activating connections. indicator-network/connectivity-
api maybe? or the APN editor?

^ This isn't done by NetworkManager. It seems to be consistently
happening after 20-25 seconds of being connected.

Please try to increasing the logging in NetworkManager (add --log-
level=debug to the exec line /etc/init/network-manager.conf), and attach
syslog again, maybe there is something to be found out from there.
