The "malformed file" appear if more than one block is used : so the
question of multiple blocks

The actual grub used (2.02~beta2.10) has not having that issue when kernel 
prior to 3.15 is used
That issue appear with ext4 partition

So this seems related to how the recent kernels now work with ext4 and
how grub do. Browsing the grub upstream list bugs, i feel the main cause
is reported as : http://savannah.gnu.org/bugs/?42466

and the new ext4 kernel rule:
