Solved my problem.  There were two partman-
auto/init_automatically_partition lines in the default
file://cdrom/preseed/ubuntu-server.seed preseed file (not in my initrd),
I commented them out and now it works.  I assume you could just remove
this preseed file from the isolinux.cfg/txt.cfg kernel params and
incorporate any needed lines into your initrd preseed.cfg.

# Suggest LVM by default.
d-i    partman-auto/init_automatically_partition       string some_device_lvm
d-i    partman-auto/init_automatically_partition       seen false
