I have exactly the same issue after upgrading from 13.04 to 13.10 - missing debugger helper, and a warning appears when running a program in debug mode. Exact same package versions, etc., as b3nn0.
I wonder if the issue has to do with migration from Qt4 to Qt5 libraries? /usr/bin/qmake-qt4 is a symlink: qmake-qt4 -> ../lib/x86_64 -linux-gnu/qt4/bin/qmake. I also found qt5 under /usr/lib/x86_64-linux- gnu/qt5, and surprised Qt Creator is not set up to use the qt5 versions on 13.10 by default (though I upgraded using the software updater program - not an installation disc.) In Qt Creator, when I go to Tools>Options>Build & Run "Qt in PATH" is selected (location /usr/bin/qmake-qt4). At the bottom "Qt version 4.8.4 for Desktop". When clicking "Details" beside "Helpers: QML Dump", "GDB Helper" says "not needed" and all buttons are deactivated.
