I can confirm this bug on Ubuntu 12.04 3.2.0-38-generic #61-Ubuntu SMP
x86_64. Terminal output is almost the same as reported:

borislav@borislav-desktop2:~$ gnome-phone-manager

(gnome-phone-manager:27436): Bluetooth-CRITICAL **: 
bluetooth_chooser_button_set_property: assertion `str == NULL || 
bluetooth_verify_address (str)' failed
