After I filed this bug report, I got some indications that the motherboard was failing on the computer. I swapped out the old motherboard (MSI N1996) for a new one (ASRock 970 Extreme3) and tested the device again. It is working fine with the the 3.5.0-22 kernel. It looks like the problem was caused by a failing motherboard.
