The problem here is that the default set of icons "andromeda" are not installed. When they are installed they are located in:
/usr/lib/openoffice/share/config/images.zip So what needs to happen is either install them by default or change the icon fallback routine to fallback to an icon set that is always going to be installed, eg human.
