Joseph, yes every time was a indeed a fresh install.  I never attempted
to disable the NVidia card in the BIOS.  I did however, attempt the
opposite, enabling optimus and using the nouveau drivers.  People
suggested they had luck with that, however I remember not succeeding.

Currently, I am working with 12.04.  My current solution has to been to
stay booted once I boot successfully... I've noticed higher success with
the following setup:

  1) nomodeset - once when Nvidia's drivers are unused (during a kernel
upgrade for example).  It's important that I remove this once the
drivers are compiled and installed for the new kernel otherwise I do not
boot.

  2) Empty boot lines for the kernel, in other words removing "quiet
splash".
