Reverting to Precise's bluez did not eliminate the problem.

However, when I booted into my last Precise kernel (3.2.0-32-generic) my
Bluetooth became as reliable as it was before. This is even with the
latest bluez (4.101-0ubuntu6).

So, this is not a bluez problem, but a regression in Linux between
kernel 3.2 and 3.5.
