For the last year or two, I have been using Ubuntu in a triple-head
setup: 1 laptop screen, plus two DVI monitors. This hasn't always been
glitch free, but my recent experiences have been fairly smooth*. I'm
using an ATI card with the 'radeon' driver (lshw reports "Whistler XT
[AMD Radeon HD 6700M Series]").

Yesterday, the login screen appeared on only two of the three screens,
and Unity appeared to hang whilst starting up, again only showing on two
screens.

After some fiddling I determined that the blank screen's DVI cable has
to be physically unplugged for Unity to start correctly.

Some fiddling further showed that using a VGA cable for the blank screen
allows it to boot to desktop, but I still only get two screens. However,
I can now choose which two I want.

I suspect that this was related to the update I did the day before. I
have attached a screenshot of my update history.

Footnote:
* It was still the case that the "displays" dialog sometimes failed to bring up 
all three screens correctly, but, once set, the triple display setup worked 
very reliably at log-in.
