I'm quite sure this problem was caused by the latest SRU as it contained
some backported fixes from Quantal where similar problems have been
observed (bug 1042024). And those problems have now started to occur in
Precise too.

Could you downgrade Unity to 5.14 and test if the problem was caused by
the Unity 5.16 SRU few weeks back?

To downgrade, download following packages:
https://launchpad.net/ubuntu/+source/unity/5.14.0-0ubuntu1/+build/3676768/+files/libunity-core-5.0-5_5.14.0-0ubuntu1_i386.deb
https://launchpad.net/ubuntu/+source/unity/5.14.0-0ubuntu1/+build/3676768/+files/unity-common_5.14.0-0ubuntu1_all.deb
https://launchpad.net/ubuntu/+source/unity/5.14.0-0ubuntu1/+build/3676768/+files/unity-services_5.14.0-0ubuntu1_i386.deb
https://launchpad.net/ubuntu/+source/unity/5.14.0-0ubuntu1/+build/3676768/+files/unity_5.14.0-0ubuntu1_i386.deb

Open terminal and go to the directory you downloaded those packages to (for 
example if you put them to Downloads run following):
cd Downloads

Then run
sudo dpkg -i *unity*.deb
unity --replace

If the problem is gone, it's definitely caused by the latest SRU.

When done, run following command to install the latest Unity:
sudo apt-get install unity unity-services libunity-core-5.0-5 unity-common
