I have never been able to fix this problem. It's there "ever since", and it's still here also with the latest distributions/kernel/ALSA (tried with kernels up to 3.2.5 and latest ALSA).
That is: the bug also affect *Ubuntu 12.04 Precise Pangolin, and likely will affect 10.10 too. As a partial good news, there is a workaroud: if you suspend or hibernate the machine (s2ram or s2disk), after resume you'll get audio from the internal speakers too. Quite unconvenient, but better than nothing.
