I fixed it by changing the model used by alsa. I don't use the
microphone, so I haven't tested if it is affected by changing the model.

Could you try to add (without quotes)
"options snd-hda-intel model=z71v position_fix=1"
at the bottom of the file alsa-base.conf.

Use the command
sudo gedit /etc/modprobe.d/also-base.conf
in the terminal.

Afterwards restart ubuntu. Your headphones should be working again.
Could you check whether the other ports (line-in,  internal microphone) also 
continue to work properly?
What about the external microphone?
