Thank you for your e-mail, I apologise for delay in replying but I’ve been 
away.

I am pleased to inform you that I have sorted the problem. I remembered that 
I had changed the appearance background settings (Theme) from Ambiance 
(default) to High Contrast. I clicked back onto Ambiance or Radiance and the 
settings icon came back straight away onto the right hand side of the menu 
bar. It seems that using High Contrast or High Contrast Inverse it takes the 
settings icon away from the menu bar. Perhaps this will teach me to stop 
messing around with default settings.

I am glad to say everything seems to be back to normal.

Are you using the new Unity environment, or the GNOME Panel classic
environment?

In other words, do you have a left-side launcher on your screen?

Bug description:
  Since updating my laptop to ubuntu 12.04 LTS, I have lost the settings
  icon to the right of my name in the menu bar, and thus I cannot shut
  down or restart my laptop.
