apport information

** Tags added: apport-collected precise

** Description changed:

I upgraded 10.04 to 12.04 beta and have run update and upgrade in
  terminal.

The app bar on the left has missing icons even the the name is there and the
app can be run by clicking the blank space.
The system button, that I believe should be on the top right next to my user
name, is not accessible. My only shutdown option is to press my power button
which brings up the shutdown options box.
Many windows once open don't appear to have a minimise or close option.

- I accept that there are going to be differences with Unity but these are
- basic issues for a supposedly LTS!
