Public bug reported:

My HDD failed and I'm using an older replacement HDD to get my machine
back up. I ran the installer once, when it was done I removed the CD and
re-booted the machine. I interrupted the boot process to change the
sequence back to the HDD, and let the process complete. When the machine
came back up it had a "Load Error" message in the top left corner of the
screen, and that was all. I tried to re-boot the machine again, and it
came up with the same error.

I re-started the machine again, interrupted the sequence again so that
it would boot from the CD-ROM. Once the machine was running on the CD I
tried to re-install the OS. About half way though (or so) is when I got
the OS Crashed message.
