Where I've seen this sort of thing (and I'm sure it's buried in the
dupes) is on postfix package installs.  Perhaps it would be worth while
to examine it's use of debconf and see if it's doing anything
troubling/likely to cause such problems.  That might lead to a hint for
the more general case.
