I have bug like this on my Lenovo t420. Sometimes after a wake up I can't 
connect to any wireless network.
In these cases dmesg shows a lot of messages like this:

[43266.792244] rtl8192c: Loading firmware file rtlwifi/rtl8192cfw.bin
[43267.979418] wlan0: direct probe to 00:1f:c6:3d:37:f0 (try 1/3)
[43268.176717] wlan0: direct probe to 00:1f:c6:3d:37:f0 (try 2/3)
[43268.376160] wlan0: direct probe to 00:1f:c6:3d:37:f0 (try 3/3)
[43268.575719] wlan0: direct probe to 00:1f:c6:3d:37:f0 timed out


or like this (in these cases it asks for a password for a network but still 
can't connect)

[43309.227439] wlan0: authenticate with 00:1f:c6:3d:37:f0 (try 3)
[43309.426985] wlan0: authentication with 00:1f:c6:3d:37:f0 timed out

I've figured out that workaround is to reload wi-fi modules:
$ sudo rmmod rtl8192ce
$ sudo rmmod rtlwifi
$ sudo modprobe rtl8192ce
$ sudo modprobe rtlwifi

So, I suppose, that the modules just do not return to the proper state
after a wake up.
