I note this behavior on some pages that have flash content. It's as if
the flash applet has the "focus" and the parent tab (window) doesn't see
the CTRL+W keypress.

In addition CTRL+W doesn't work if the page has not yet loaded. For
example I have a bookmark folder that has a number of sites listed, I
open all of them in tabs (only about 6 pages) then read them and close
them as I go with CTRL+W. If i get to a tab that is having a problem due
to the server not responding, CTRL+W does nothing. I have to press ESC
to stop that tab from attempting to load, then press CTRL+W to close it.
I was going to report this as a separate bug, but it seems related to
this one.
