The "differently coloured" triangle refers to Firefox being on *another*
workspace, not the current one.

>From what I'm guessing, the issue is occuring because multiple Firefox
windows are open (or probably one real and one ghost window), and one of
them is getting matched properly by BAMF, while the other (or the real
window) remains un-matched and that's why clicking the firefox launcher
icon does nothing. And that's why the real window doesn't have any
menus, title or window buttons.

Do you note a pattern with why this issue occurs? Does it occur only
with firefox? Does it occur when doing only certain tasks (running a
download, etc) ?
