I went over the duplicate cases and they all (well, both) appear to have
ext4 (after I pruned the ones which were not all that similar after
all).  They also all seem to me multiple-CPU systems.  Bug #643016 shows
in dmesg.txt that ext4 recovery removed some unreferenced inodes, but
that might be a red herring.  Another data point is bug #782033 which
however was closed as Invalid, but it also seems to match the symptoms.
