I have been affected by this in both 10.10 and 11.04. I was hoping it
would be fixed in Natty, but since it's not, I tried both solutions in
this bug report.

Editing /usr/share/pulseaudio/alsa-mixer/paths/analog-output.conf to
change the Element Speaker entry to volume = merge and rebooting didn't
work for me.

Editing /etc/pulse/daemon.conf and commenting out flat-volumes did the
trick.

This particular machine uses a digital optical output, not analog; I'm
not sure if that's why the analog-output.conf didn't work or not.
