I've just tested this in a new install made with the i386 RC ISO image under VMware (ISO hasn't changed in a couple of days, synced with rsync). The bug is still there.
I think the bug description is not clear on how to reproduce the bug. I'll rince and repeat for the sake of clarity: 1. Open OpenOffice.org Base. 2. Create a new database, save the file. You'll be droped into the Form window. 3. Click on the Tables button on the left. Click on the second option in the upper section of the Tables section when it appears: "Use wizard to create table". 4. Go with the flow: Add all the fields offered by the default template using the ">>" button, Keep clicking "Next" until it deactivates. Select "Create a form based on this table" and click "Finish" 5. The Form Wizard opens up and loads the modal dialog. Copy all the available fields to the form with th e">>" button. Click "Next" and keep clicking it until it deactivates. 6. Click "Finish".
