A bit more information on this bug.  It appears this only happens when
"Mirror screens" is enabled - which is the default behaviour.  So
anytime you plugin a new external display or change an existing display
to mirror screens you can trigger this bug.  I didn't see it on my
external monitor on my desk because it was setup as an extended screen.
