Confirming since the previous bug already mentioned such an issue. Seems like the device gets set to an unusable mode once the connection fails, so this may be caused by driver issues. Could you follow the steps at https://wiki.ubuntu.com/DebuggingModemmanager to provide debug logs for ModemManager?
