A warning should be popped up during installation if the mysql server is not accessible during installation. The user will have to run dpkg-reconfigure mythtv-database in this case. It doesn't automatically pop up because then during upgrades users would receive errors about mysql passwords not working (because mysql server is stopped during an upgrade) and are likely to further muck up things by investigating wrong passwords.
