Public bug reported:

Binary package hint: flashplugin-nonfree

pulseaudio appears to work fine with all application except flash. but
in flash, no sound is produced. in the applications tab for gnome-
volume-control, it appears to show up as: "alsa plugin [ plugin
container ]", but the entry flickers. i assume that this means that it
is starting, exiting, and restarting ad nauseum

i've run pulseaudio with the -v and --log-level=4 flags, and i see and
endless repeat of: created input, lots of info, freeing input, freed
plugin, connection died. these loops are happening at 20hz (eg, 207
occurrences in 10 seconds of youtube). here's a clip of one of the logs:

I: sink-input.c: Created input 169 "ALSA Playback" on
alsa_output.pci-0000_00_14.2.analog-stereo with sample spec s16le 2ch 44100Hz
and channel map front-left,front-right
[skip]
D: source.c: Processing rewind...
I: sink-input.c: Freeing input 169 "ALSA Playback"
I: client.c: Freed 170 "ALSA plug-in [plugin-container]"
I: protocol-native.c: Connection died.
